Health Goals

Towards the end of the onboarding process, you will receive an overview of the app, followed by the opportunity to choose your main current health goal from a pre-populated list of topics. Depending on the choice you make, the app will present you with the tools available to provide the support you need to meet your goal (including Care Programs, Reminders and Secure Messaging with your dedicated Care Team). 

Choosing a Health Goal

Once you have browsed the brief app overview screens, the app will ask you “What is the main health goal you would like support with” and present a scrolling list of pre-populated health goals to choose from (you can only select one). 

NOTE: If you do not wish to choose a health goal at this time, you can tap the X on the top right corner of the screen and you will be taken to the next section, “Setting a Reminder”. 

To choose a health goal: 

  1. Scroll through the list of health topics and tap on the one that you believe is your main health goal 
    NOTE: If you select “Weight Loss” as a goal, you will be brought to a screen that prompts you to answer a question before proceeding 
  1. You will then be presented with the list of tools that Wellframe has available to support your health goal 
  1. Tap “See Care Programs” at the bottom of the screen to see a list of the programs available for your health goal 
  1. Tap each of the listed care programs to view further details, and then if you decide to enroll, tap “Request to Enroll” at the bottom of the program description screen 
    NOTE: If you do not wish to enroll in any of the care programs, tap “No Thanks” at the bottom of the list of care programs 
  1. Once your request has been sent, tap “Continue”, and you will be taken to the “Setting a Reminder” section
