Glucose Monitoring

Members with diabetes may be able to participate in Wellframe Diabetes and Blood Glucose Biometric care programs. In the app, you can monitor your blood sugar and access insight into trends to discuss with your care team to better manage your blood sugar. If you are enrolled in a Blood Sugar Monitoring program, you can also sync your blood sugar readings into the app: from Apple Health on iPhone, or from Google Health Connect on Android.

Connecting to Apple Health (iPhone)

(Optional – applicable to iPhone users).
  1. After you are enrolled in a Blood Sugar Monitoring program, you will have a checklist item to record your blood sugar reading.
  2. Select that checklist item to display the entry page, where you can either manually type in your blood sugar reading or select the Sync option.
    • If this is your first time recording a blood sugar reading, select Sync, and a page displays that says Connect Now.  
      Note: If you do not wish to connect to Apple Health, select No Thanks
    • After you select Connect Now, the next page displays the Apple Health option. 
    • Select Apple Health to display the Health Access page. 
    • On the Health Access page, turn on the Blood Glucose toggle to enable Blood Glucose data to sync. 
  3. On future blood sugar checklist items, select Sync readings beneath the entry field to display recent Apple Health readings, then select a reading to populate the survey response. 

Connecting to Google Health Connect (Android) 

(Optional — applicable to Android users. Requires a supported Android OS version with Google Health Connect installed.) 
  1. After you are enrolled in a Blood Sugar Monitoring program, you will have a checklist item to record your blood sugar reading. 
  2. Select that checklist item to display the entry page.
    SYNC READINGS button displays beneath the entry field. 
  3. Select SYNC READINGS.
    Note: If this is the first time you are syncing on this device, a permission education dialog displays before the system permission prompt.
  4. Grant Health Connect permission to read blood glucose data when prompted. 
  5. After permission is granted, a list of blood glucose readings from Health Connect displays. Readings from the current day are shown with the most recent at the top. 
  6. Select a reading to populate the survey response. The reading timestamp and data source are saved with the entry. 

Tip: You can manage Health Connect permissions at any time from the Blood Glucose section in the Wellframe app Settings.
