You may be asked to enter information during the onboarding process to pass an eligibility check depending on your sponsoring organization, you will either need to enter:
Member ID and Date of Birth or Last Name and Date of Birth
Don't worry, the app will let you know what information you need to enter!
Member ID: The Member ID is located on your health insurance card. For more information on what portion of your Member ID to enter, please tap the link to "Where is my Member ID?" or "Where do I find this?" in the app directly below the Member ID entry field for specific instruction.
Last Name: Enter only your last name. If you are encountering any issues with passing the eligibility check when asked for your last name, please reach out to your care team to confirm that your information has been entered correctly into the Wellframe system.
Date of Birth: To enter your date of birth, enter your birthday into the calendar picker on iPhone and Android.
*Tip for Android Users: if you are unable to skip to by year when selecting your date of birth, try tapping directly on the “1950” in the calendar picker window to skip by year rather than by month. This link has more information on how to do so!
