Wellframe is a secure platform that allows you to connect with a member of your care team at anytime and allows your care team to track your progress to further personalize your care plan. By completing the tasks in our daily Checklist, such as surveys, educational articles, and medication reminders, your care team can get to know you better to further customize your care plan for your needs.
You can contact a member of your care team at any time by using the “Chat” tab on the app. Here you can ask your care team about your care program, medications, or other health questions. Please note that your care team may not respond immediately. If you need to speak to a doctor or in case of an emergency, call 911. Wellframe should not be used in emergencies.
If you would prefer to talk with your care manager on the phone, simply send your care manager a message from the Chat tab letting them know you would like to schedule a phone call.
Who is on my Care Team?
Your care team is the person or group of people on the other side of the App helping you with your health. They may be a nurse, case manager, social worker, community peer, or other health worker. Your care team is there to monitor your progress, message you with updates, and answer when you have questions.
Through the app, you may have access to a Health Advocate (also referred to as an Advocate). Your health advocate works with you and your care manager to support a healthy lifestyle. Your health advocate is not a doctor or a nurse, but they are trained to assist with any non-medical questions or goals while you work to manage and improve your health.
Through the app, you may have access to a Support Specialist. Your support specialist is there to help you with any technical support or guidance you may need while using the app. Your support specialist is available via phone or messaging.
